Here's a picture of the plywood driver I made (more pics at the link below). Note that it is not necessary to chisel out the small circle in the middle.
From all I've read/been told, on the flywheel side, oil typically leaks out where the end plate meets the head, and at the rear seal. I don't know how you'd be able to tell where it's coming from without at least taking off the clutch cover. Even if the seal is dry, I'd replace it anyway because it's a rubber part that's likely 10+ years old.
