If up and down shift OK as you have described, on highway listen for whine in 3,4,5th gears. Usually easiest to hear in fifth.
If clutch pedal engagement is well off the floor but it still crunches into reverse, may just be a bad pilot bearing(supports clutch shaft in center of flywheel) or a dry clutch shaft.
