I have had similar symptoms from time to time. Sometimes it's a bad Air Mass Meter causing surging and flat spots even before boost takes hold and continuing to limit power under boost. If the plug gap is too wide or the dist rotor is bad, you can get misses/hesitation under high boost. Another cause can be if the base timing is significantly retarded from 16 BTDC.

Timing issues can also appear if the vacuum advance/boost module on the side of the distributor has been overtweaked (bending/grinding the limit post too much). The symptoms are high boost with no knock intrusion, but also reduced power. If this limit post has been altered, base timing has to be advanced to compensate for increased retard under boost.
