Yeah, it happens. That is why I use redundant jacks. My brother-in-law and I are not small people, so when we pull on a ratchet, the car bounces. If we repeatedly pull in a rhythm, the car's body begins to sway. We try to avoid that because a swaying car is a deadly one. We jackstanded the car in the front in 2 frame places where one would place a Saab scissor jack. We placed wooden chocks behind both rear tires. The car was an '86, so emergency brakes were useless as they were on the front. The asphalt street we were on had a slight roll down on the passenger side for water drainage. When we worked from the top, we kept power pushing the ratchet and I guess we developed a rhythm OR we just pushed hard (we were both pushing - that makes 1/4 ton of people power). The front car tires were only off the ground by 1 inch thank Heavens. That is minimal length for a jackstand. They are expensive and wide pyramidal base jackstands. A jack that had been firmly planted toppled. We did not investigate whether or not the tire chocks in back moved. Rather, we stopped working and re-assesed ourselves. After a break we decided not to jack the car up, and we finally got to the difficult bolt from an awkward position in the top.
The moral of the story is now when we jack a car, we place a redundant pair of cinderblocks beneath the engine crossmember in addition to the jackstands and the wheel chocks.
