is the cable coming off, or is the pulley shifting down and causing the cable to slide off?
when i was instructed how to fix mine, i was told to drill through the pulley and the shaft and insert a cotter pin so the pulley stays in place. it is just slid on there. so far mine has held since the repair. i went tight on the cable as well.
there may be an issue with the locknut. you may want to get a new one to make sure.
