Yes...cap can fail to high pressure. OTOH, it sounds more like you're heading toward a leaky head gasket if not already there. Hie thee to a shop or dealer with an exhaust gas analyser and have them sniff the expansion tank with the cap off...engine hot, etc. Head gasket leaks leading to high pressure will generally be detected as HC coming out of the radiator coolant.
13.0 voltage may not be wrong although I would expect 13.2 with a stock bosch voltage regulator. IPDUSA.com sells an adjustable regulator that works in bosch alternators...I have one on my volvo set to 14.2volts. Putting it in a saab is relatively easy.
Are you sure you're measuring the output voltage of the alternator? Use DVOM at the alternator, not at the battery. Possible causes of voltage drop are the battery cables, ground, and/or wire from the alt to the battery. Voltage drop across cables can only be measured while the cable being tested is under load, i.e., carrying current, since V=IR.....you should ideally have 0.0v voltage drop across any cable.
Also, even a new or fully charged battery may be deficient, check the individual cells for proper specific gravity if possible...load test or have the battery load tested....it should maintain 12.0 volts at 300 amps draw for 5-10 seconds. Or just turn on the high beam lights, and let them run for 10 minutes or so (engine not running)....voltage shouldn't drop below 12.0 volts. If it does, the battery fails the load test and should be replaced with one that doesn't fail.
