. . . almost certainly a brake rotor that is warped. Does this tend to be most apparent when braking on a freeway off-ramp? If yes then its most certainly rotors.

There are a couple of possiblities for the cause of this:

1. The rotor is too thin from wear or from previous machining and thus prone to deformation

2. Incorrectly torqued lug nuts- some more tightened than others.

3. Areas of brake pad deposition from incorrrectly bedded-in pads.

Can't say for sure whether it is the front or rear rotors however i would suspect the fronts given that they do most of the work and are directly connected to the stearing components (thus the shaking in the wheel).

The fix: new rotors. If the pads are significantly worn this would be the time to change these as well. Try the oem brand rotors: zimmerman, brembo etc. Make sure you observe the correct procedures for bearking in the new rotors (just liking breaking in new pads) and torque the lug nuts evenly.
