Consider how often the driver side door is used - all the time! My door has rattled since forever. While replacing the interior door panels some time ago I went looking for the rattle source. There was one obvious possibility: a crack in the door chassis where the auto window regulator assembly is bolted to the door with four 10 mm bolts. The forward bottom bolt hole on the door had become enlarged and the metal cracked. The rearward bottom bolt hole had also become enlarged, but had not cracked. My solution at that time was to realign the regulator assembly as best I could and re-tighten all the bolts. While more secure and less noisy, it didn't stop the rattle completely, especially when shutting the door.
Over time the noise increased and then one day last week when I let the window down I heard a clanky shifting noise. Time to pull the door panel again. What happened was besides the cracked chassis issue, three of the bolt holes in the door chassis had enlarged to the point where they could no longer grip the 10mm bolt head. Only one bolt was actually holding the window assembly to the door! The solution this time? I used three washers large enough to extend well beyond the area of wear, and with center holes the perfect size for a 10mm bolt shaft. Door is now rattle free.
