If it's like my 91, the telescoping height-adjustment lever has two small nubs on its plate, that insert into two holes on the seat frame, centering the lever/rod/counter-spring in the seat farme through hole, and two small/short fine-thread bolts (10 mm head, IIRC) that hold the lever flush to the seat frame (bolt heads are on inside, not outside of seat frame base).
If either or both of the two bolts has loosened/fallen out, could be causing your problems
