For a couple of days before you are going to do this it's a good idea to spray the nuts and bolts with penetrant. The welded nuts that the bushing mount bolts go into can snap their welds. All is not lost because you can just use a regular nut if they break off.
With the car on the ground take off the end nut on the crossbolt. Then take out the bolts holding the bushing mounts, keeping track of the shim positions. With any luck the bushing mount will slide off the big crossbolt.
If the bushing sleeve is rusted to the crossbolt, you can try more penetrant and whack the mount off with a hammer. If still no good, you can get out a propane torch. If the rubber bushing gets destroyed in the process, that's OK because you can now get the mount off and go after the sleeve separately with a vice-grip or a dremel.
Then you will need a press, vice, or socket/bolt makeshift tool to force the new bushing into the mount. I have torn these bushings so I am now very careful to heat them up in a cup of water in a microwave oven prior to pressing. Then soap up the mount and bushing, perfectly center the bushing over the mount, and press it in.
