All the issues may be related to the reverse lock-out pin setting at the end of the shifter. Fifth and reverse become a problem when the tip is out of spec.
First, remove shifter boot and inspect the three torx bolts that hold the shifter ball in the housing. Those can sometimes come loose and cause 5th/reverse problems. Also, you want to clean and relube the lower end of the shifter components with white lithium grease. I think the setting for the length of the tip is 5mm, but don't quote me. If you have a bentley, they have a good right-up on adjustments and part replacement. You can leave the car in neutral and remove the three screws to clean and inspect the unit.
