I tried to do a search on this but my write-up must be too old. It happened to me with my 91 900NA in Dec 96 at 2 am on our way from the LA area to Colorado. About 4 months prior to that I had my HB replaced in Colorado. After driving it a little I started to notice that the car was getting sluggish and I had this tapping noise near the pedal cluster at idle. I brought the car twice to my mechanic in SoCal and he did not find anything. On our way to Colorado, the car got worse and worse, and then the temperature started to go up a lot. When I stopped the car along the road just before Needles AZ, the exhaust manifold was glowing bright red in the night. We got the car towed to Needles and a local shop looked at it there and could not find the problem. So we put it on a flatbed and had the car transported to Hardy in Las Vegas. The mechanic there (Myron Judkins) was exceptionally good. One of the first checks he ran was the verification of the timing and he immediately knew there was a problem with the HB because the ignition was way too late. He took off the belts and found the loose HB, just like yours. For my HB, it actually had started to crack around the key area and the key itself was completely rounded. He managed to remove the key from the shaft and replace it without taking the engine out (I told you he was good). According to him, there was a spacer missing. When you take of the originally installed HB and replace it with a new one, you have to add this spacer. Just swapping the new part for the old one does not work.

We left the car with him and continued our vacation with a rental and picked it back up on our return trip. His bill was about $500, which included also the FMS an unrelated CV joint repair. My total bill was about $1000, including both garages, rental car, night hotel in Needles, and towing charges. The garage that had done the work originally volunteered to pay half of the total bill.

So going back to your questions:
1) Is there a shim between the nut and the pulley? Yes. It probably was not there for the original one but had to be added to the replacement (according to Judkins)
2) If there is a shim do they split and fall out? I sincerely doubt it. I just did my HB last month myself and this spacer is very thick. I would be really surprised if that would break.
3) If there is no shim - what else would cause the crank pulley to wobble and enlarge the keyway? The bolt is there to hold the HB tight to the shaft. Without the spacer, the bold will run to the bottom of the hole in the shaft and not hold the HB tight. Just a little movement will develop in a big problem over time.
4) Is eight hours reasonable to diagnose the loss of engine power cause by the wobbling crank pulley? (They checked fuel pump, filter, injector pressure, and replaced the ignition computer prior to my asking if they had put a timing light on the crank? The next day they told me about the wobbling pulley.) No way. Ignition timing should be one of the first diagnostic tests. It takes no more than 5 minutes and you can rule out a host of problems by doing this.
5) Should I have to pay $653 of the $1153 bill to diagnose and then repair this problem? That level of effort is a consequence of their incompetence. You should not have to pay for this.
6) Should I now have to pay another three hours labor to replace the oil pump seal? I'm not clear what you are referring to. There is the oil pump seal which is a long o-ring and that one should not have been affected by this problem. However, there is also the front main seal (FMS), which is seated on the oil pump cover and actually seals on the HB. If the HB had a lot of movement, I would not be surprised to see some serious damage to the FMS. They should have replaced it at the same time anyhow as a preventive measure. Anytime you take off the HB, you might as well do the seals at the same time since most of it involves labor to take off the HB.
7) What are the chances the keyway in the shaft that the pulley attaches to is damaged? Hard to say without looking at it. Depends how bad your problem was. If you send me an e-mail I can probably send you a picture of mine. I still kept it as a reminder.
