Au contraire, MS, as they say in la belle France, the throwout bearing SPINS WHEN the clutch pedal is depressed to the floor, under quite a heavy load in fact. It should NOT SPIN when pedal is up, whether in neutral or driving in any gear, though it might spin minimally from minor non-pressure contact with the tips of the fingers of the clutch diaphragm.
It IS a suspect here though - - it could be making its noise when idly spinning in gear, but under the heavy load of doing its job it might tend to quiet down. If so it's probably about to go bad.
It doesn't seem as if it could possibly be the pilot bearing, as that spins only when clutch pedal is depressed but can't possibly be spinning while you are driving in gear as the input shaft, the clutch disk, and the flywheel are all three spinning at the same speed, locked together.
