Definitely the front mount and passenger side mount. I've done them with the same basic jack. Some other guys (I think it was either Dave New or GM) have posted about replacing the driver's side mount using a jack. I can see how it can be done, but I haven't braved that task.
Replacing the front mount using the jack is pretty straightforward. The most difficult part for me was moving the fan (with those rusted little torx screws) that's directly in front of the mount. You'll want to use a piece of wood or hard rubber to protect the part of the transmission that you use the jack on.
My experience with the passenger side mount is that you'll want to disconnect the downpipe from the turbo and from the transmission bracket to get a little more lift to clear bolt heads, etc. on the mount and the riser that's bolted to the "frame" rail. I found that there's *just* enough clear room get it out without putting too much pressure on the shift shaft seal if you don't disconnect the shift shaft coupler. Same thing as far as using a piece of wood or hard rubber to protect the part of the transmission that you have the jack under.
