Heater Hose Question

I have a '90 TC automatic. One of the heater core hoses (upper?) has a smaller diameter branch coming off of it and it leads to another small diameter hose, just underneath the intake manifold, leading to the fromt of the car. It ends on a metal junction, but it doesn't look like anything coolant related...what exact is this leading to? If I were to replace this heater core hose, what is this other smaller diameter hose? It looks like its a rubber-to-rubber join with no metal junction; is there a metal sleeve inside both hoses?....only one clamp is visible. Can this smaller hose still be purchased because I never see any reference to it?
These are the original hoses with date stamps of 1989. They are still soft and pliable. Should I be concerned with replacing these as they are 15+ years old? This would also include that long lower radiator hose, but some have said that this appears to be a hose that does not fail. would you change them over to new. Opinions?
