The 91 box has the internal centering spring/ball bearing design. It's accessable when the right side panel on the transmission is removed. It's the panel which includes the dipstick hole. I can't imagine how that could have gone bad since it's a simple design with little to go wrong. Unless that spring was never inserted in the transmission during the rebuild/replacement and you've been using the older console centering spring. My advice then would be to insert that internal centering spring and ball since it's much better at centering than the older version. The older console spring design can fail, yes, and it's a matter of reattaching the spring, or replacing it in the event it broke in two. You can access it by pulling back the carpeting (one of the seats may have to be removed to do so), remove the heating duct which covers the spring unit, then use a long needle nose pliers and reattach.
