First off, it is very unusual that a tooth break off but it can happen.
When rebuilding transmission, ALL gears and bearings are NOT replaced on a regular basis.
Only the parts that looks worn or parts that are known to break or wear out gets replaced in a rebuild.
That means that most to all cog gears are still the same after the rebuilt is done, which means they might have 100-200K on them in a newly rebuild transmission.
So it could very well be that this 1st gear tooth finally gave up its ghost due to a tiny fraction (or other weakness) that could have been in there ever from the transmission left the factory many years ago.
Or a rough start (clutch pedal slip) could have taken out the gear.
Unfortunate but it can happen.
