If the cotter pin was out, then likely the reverse-lockout pin adjustment is off. The cotter pin not only holds the lower ball on but it also keeps the reverse-lockout pin from unscrewing. The r-l pin is the pointy thing which extends out the bottom of the shifter below the lower ball. It screws on to threaded shaft that connects to the reverse-shift lever. It should protrude about 7/8" from the base of the shift rod. If that is not the distance that yours is protruding, that is most likely your problem. You can adjust it by removing the cotter pin again and turning the r-l pin until it is the correct distance, then reinstalling the cotter pin.
