It could be two things.
The steering column U-joint is sticking and that would be felt every turn of the steering wheel one time per revoloution. You would also feel that if you lift the car up in the air and turning the steering wheel with engine off.
The lower part of steering shaft needs to be replaced.
The other problem is a worn steering rack where it develops "morning sickness".
The steering is hard to turn (especially to the right)and when you apply more force it suddenly let loose and the power kicks in. Those symptoms usually goes away when the car gets warm and comes back when cold again.
A new steering rack is needed for that.
