this is a bit on the danger-side
when the car is hot or warm, with the motor off try to unscrew the radiator cap, looking to see if it has built pressure
only need to unscrew it a little to see if pressure escapes,
if there is no pressure you got major problems,
if there is, or you get a 5th degree burn everything's `A` ok

if there is no pressure when the car is hot and motor is off look at all other fluids, look to see if the oil has bubbles and has more than the amount it should have, same goes for the gearbox, or trans.
while your at it might as well look at the level of your brake fluid and power steering,
if there is pressure and you managed not to get burned, well done (clap,clap,clap) go ahead and look at the rest of the fluids if everything checks out, change the radiator fluid,
in changing the fluid you must use the bleed screw to get all the air out of the system when filling with new fluid, the trapped air can do more damage,..worse than the 5 degree burns on your arm.
the sludge may be a film left from a prehistoric coolant that lined the inside walls of the radiator hoses, which is now just flaking off.

now about your questions 1-3

1)only after doing this test can you know if everything's ok.
if it turns out everything is the way it should be, and the sludge you see in the coolant look identical as before it very well may be, the shop you took it to only changed the rad. fluid and charged you for a head gasket.

now how could all this be done (because the car was over heating)
think way back, to the start of it all, before the car overheated the first time.
had you taken the car into the shop for a check up, brakes, fluid change, anything,...
one can disconnect the fans, or one fan, open the bleed screw to induce air of the radiator,. these are just a few things that can be done to get the car to over-heat,
depending on when the driver notices the car is overheating is how much damage it will take, if the car was redlining for an hour you could fry the head gasket or even a half an hour, if it was noticed within the first 5 to 10 minutes and the car was shut down the head gasket could be saved.
and when the car was brought back to who ever, the fix could be simply done.
this seems like a lot just for someone to do, and why would someone do it, well .. i think we all know, you most of all seeing this is the second time.

2) only after doing the tests can you know if the sludge is the remnants of prehistoric coolant, it may just be food coloring
or not, there is no way of knowing until a simple check over is done.

3) same as 2







no just kidding, the sludge has me thinking,.. last time the car overheated, had you saw sludge then,
if so was it before, or after the car over heated,
it may just be harmless prehistoric slim or tar, draining the system and back flushing, or doing a change to the fluid once a week for the next four weeks should do the job of cleaning it out.
this is only to be done if the car has pressure in the coolant reservoir and all fluids check out (no bubbles and none of the fluids are over filled)
