It may not just be on the surface. You need to tap the whole area with a hammer and break free as much as possible of the rust that's growing on the back side of the sheet metal. Whole areas that look relatively sound can crumble away to lace as you do this. Then cut and grind till you've got sound fairly bright and rust free metal, and treat with an acid etch and then paint with a really good primer. I've been told PPG DP420 is what restorers use.
It can get really bad along the wheel arches as that plastic trim holds moisture in, then it gets rusting between the inner and outer fender sections and both rust away to nothing.
If it's relatively local you could probly get away with knocking and grinding out, priming, then filling the missing area in with bondo, gorilla-hair bondo for big gaps, priming again real well and paint, then undercoat well and also re-seal the trim strip if you re-apply it, with a soft wax - - someone suggested toilet bowl wax sealing ring wax.
See DougM's recent posting of a photo of a wheel lip repair he did welding in sections cut from a junker. Impressive but if you're in relatively dry clime and if it's small still, probably not needed.
