I have experienced the problem you are describing. The upper portion of the shaft (ie. everything above the 2nd upper u-joint) is actually two pieces that are crimped/crushed together. This is part of the collapsible function of the steering column. In the event of an accident i'm guessing that the two parts of the upper shaft collapse into each other and the "expanded" metal cage around it also caves in.
Over time, if the steering wheel is used to help the driver into or out of the car or it receives a bit of a hard knock (or yank!) the crimp can loosen up and allow the two parts to slide a bit in relationship to each other. I don't think it's a big problem or safety concern but don't quote me on that.
The fix requires removal of the dash bolster and steering column assembly. Not hard but a bit time consuming. The steering column is held on by 4 nuts if i recall correctly. Once you have the steering column out the problem will be apparent. You can either crush/hammer the metal around the crimp a bit to restore the original fit and eliminate the motion between the two parts (maybe only a temporary solution) OR replace the steering column with a used unit. I did the later using parts from one of my junkers and the problem was solved.
