what is this vibration?

Hello all,
A friend of mine drives a 1991 Saab 900 2.1 and sometimes he has a big vibration in the car. Just as if the road is very oneven. It comes somewhere from the front. You feel it thoughout the whole car. The vibration is not always there, and not dependent on a certain speed although at a higher speed it is felt better.
The wheels are rightly balanced and are 100% round (has been checked) and there is no play in the front axles. The brakes are OK. At all times when vibration is there, it is gone when letting the gas pedal loose. Sometimes vibration is there when acceleration, but not always. Sometimes it's just there when driving on a flat road without accelerating, and sometimes it's bad and sometimes it's not.
My friend has the impression that sometimes the engine does not run stable at 850 RPM.
Anybody got a clue what this might be? Could this be something in the primary transmission???
