There are specs for acceptable synchro wear, and they can be checked on the bench. Honestly, I think you'd be foolish to rebuild a transmission and not de-facto replace at least the 1/2 synchro, but I can see why a shop would not go that route. Still, you can check them for wear, and IMHO if it's on the "almost bad" side of the spec it should be replaced... it's dumb to bolt together a box that's expected to go another 100k (or much, much more) and not have components that will make the trip too...
FWIW, this is one of the primary reasons why buying "rebuilt" transmissions is such a rough deal. Unless you deal with a knowledgeable group like Ericksson, you often get back gearboxes that are "to spec" but still not good. My old mechanic once tore down a fresh rebuild from another company (I won't mention the name) and showed me worn primary chains & tensioners - another component that should probably be de-facto replaced. They were probably "to spec" but you know worn chains when you drive them...
