On a '91, there isn't much to go wrong - no EGR system, timing fixed. If the cat and O2 sensor are new, and you're still getting massively out of whack NOx, I would probably look towards either the air mass meter or carbon deposits in the engine. FWIW, my experience is similar to yours - put in a new, perfect O2 sensor and results get worse. I'd swing by a GM dealership and get a can or two of their Top Engine Cleaner and run it through. Does a really good job of cleaning out the combustion chambers, and might be what you need to pass.
The alcohol is also a good cheat to get you through - I usually run the tank down til the gas light comes on and add 1/4-1/2 gallon a dry-gas. E85 is hard to find around here, but probably an easier/better choice. If you can, filling up on premium U76 gas is a good call - they use ethanol instead of MTBE...
