Usually it's a ring of dried-out brake fluid varnish that makes it hard to get the piston out. I've rebuilt a bunch of clutch MCs, and that's always been the case.
As far as manufacturing is concerned, adding a setscrew would be a belt and suspenders approach (and could damage the outer rubber seal) where the C-clip is more than adequate for the application.
