Rear Shock Lower Mounting Bolt

I know this is a common issue, but I'm really quite stuck. I got the nut off the lower mounting bolt on the rear shock but absolutely cannot get the bolt out. Does the bolt thread in to the metal sleeve inside the bushing? Or should I be able to push/pull it out? I know they've got to be rusted together because I can see the sleeve turn when I turn the head of the bolt. Any help/tricks would be greatly appreciated. I've already used a ton of the PB rust destroyer and I'd really like to get back on the road. Thanks.
