You should take a look at all the things that cause a rich mixture. I suspect that the O2 sensor is seeing the rich mixture and is contimually trying to lean it out.
Start with the air cleaner, fuel pressure, dirty injectors, EGR valve, and anything else that would contribute to a rich mixture.
On the other hand, bad connections in the O2 wiring will cause a volt drop, and the computer will think that the mixture is lean (it gets a low signal) and will try now try to richen the micture up to compensate for what it THINKS is a lean condition.
Once the engine and O2 sensor are hot, then the O2 heater shouldn't come into it.
Do you have access to an oscilloscope? If you do, then disconect the O2 sensor and measure the output. You should have a square wave that oscilates each side of 0.5V.
