I'll go with the simple approach - you have worn pads. I'll assume you've done the simple thing and checked the brake fluid level.
If the pads are badly worn, they can chew up the rotors. Not only will that make some interesting noises, but a worn pad and a grooved rotor just don't stop as well. That means you push harder to get the same stopping.
Pull the wheels and inspect the pads and rotors. Don't forget the rears - just because they do less of the stopping than the fronts doesn't mean they don't wear out. One thing to look for is uneven pad wear - the inside or outside pad more worn than the other, or even one side of the rotor looking unused - usually covered with some rust. That's the sign of a sticking caliper.
And Aaron makes a good point - brake fluid should be flushed every two years. Yeah, three is OK but pushing it.
