It's very possible the fan is in water. It's not from a leaky heater core - the core is below the fan, and if it did leak that badly, you'd have puddles in the car, not in the fan box.
The fan box has a drain for any rainwater, snow, etc. that blows into the intake vent. It also collects leaves and twigs. These clog the drain, and the box can fill up with nice, swampy water. And splashing around in the water isn't good for the fan. Clean the drain, and that will help.
The ACC self-test can tell if the fan motor windings are open (no current) or shorted(too much current), but it can't tell if the fan is turning. Clean the drain out, and make sure the fan is spinning. Then you can worry about if it needs replacement. In general, you'd replace the fan as a piece - if anything is bad, it's the motor, and that's the most expensive part.
If cleaning the fan box out makes the fan work, and the fan runs quietly and happy, then you may not want to spend the $$ to replace it. But if you are going to replace the heater core, the fan has to come out anyway. And the labor for taking the fan out is only slightly less than for the core. If the core's out, I'd replace the fan while I was in there. If the fan has shown any signs of not working right, you DON'T want to replace the heater core and use the same fan, then have to do the job again in 3 months when the fan packs up.
