Balance shafts......real MYSTERY!

Opened up the timing cover on my 9000CSE LPT (B204 Engine)to replace timing & balance shaft chains, sprockets, etc. and to my amazement discovered that the existing exhaust balance shaft sprocket did not have the gear in the middle! The new replacement sprocket has the gear with 2 rings either side. If no gear, then how the heck was the balance shaft revolving...if revolving at all? The existing sprocklet does not show signs of the gear actually wearing out but the middle section has a flat ring without any teeth! Has anybody else experienced similar discovery in this carrying out this task?
