All of them should be coming on. The later 9000's have a fan in front of the condensor. I don't know if the 88 has this or not.
Also, are those pressures at idle? What are the pressures at 1500 rpms after 2 minutes? Use a rock to hold the accelerator pedal.
What is the ambient temperature?
Did you try adding more r134a? Sometimes you have to add more to get the expansion valve working right. Is the expansion valve bulb fixed to the evaporator outlet tube and insulated?
Is the foaming you are referring to in a sight glass on the gauge manifold?
The only things that will produce crazy high side pressures are a heat exchange problem at the condensor, or an obstruction in the system. If the fans are coming on, try washing the condensor with a garden hose. Use some soap and a light spray of water. You also might try adding 2.5 cans (12oz) total and see what you get for pressures. 300psi on the high side at 1500rpms is normal if it's 100F out.
