The 50mph top speed is more of a liability concern than what the tire is capable of. Obviously the car will handle differently (worse handling, particularly is transitions) and if the driver doesn't compensate and drive more conservatively there could be an accident, and since this is America the tire company could be sued. Likewise with the 50 mile limit usually posted on the tire. Anecdotaly, one of the tire companies complete Car & Driver's "One Lap of America" using four mini-spares.
97SAAB brings up an interesting issue with the differential. Running with one wheel smaller than the other is a obviously a problem with limited slip diffs as it will wear out the clutch pack (or whatever they use they days) prematurely. Someone else posted about how the Volvo Cross Country (IIRC) is having problems with this and that there is a very strong warning about driving too far on the spare as it could wreck the drivetrain. I don't believe that this is a problem for cars with an open differential (think of it as making a very very long term) but I could well be wrong and would be quite interested in hearing arguments to the contrary.
I guess in summary: be careful on the little spare, but don't be afraid of exceed it's posted "limits" if you have to.
