Yes, it is not immediately obvious. Let me see if I can describe it.
Extraction: Place the U-shaped piece so that the tangs of the fork are on the outside of the torque arm cylinder. Put the bolt through the end of the fork (put a little grease on the inside of the bolt head), then through the bushing, place the cone-shaped piece on the end of the bolt with the flat side of the cone up against the bushing, and tighten the nut snug. Hold the nut steady with a box wrench or a socket on a breaker bar. Use a socket on the bold head side with a ratchet and tighten away until the bushing is pulled into the space between the tangs of the U-shaped piece. I've found it is easier to do this if the torque arm is clamped in a vise, but you can put the arm on the floor and step on it if you don't have a vise.
Insertion: For this, turn the U-shaped bracket upside down and place the flat end up against the empty hole of the torque arm. You will notice a step cut into the U-shaped bracket for this purpose. Now put the bolt through the cone, through the new bushing, into the torque arm, and finally through the hole in the U-shaped bracket. Put the nut on the end, hold it steady with the wrench and crank on the bolt on the cone end to pull the new bushing into the torque arm. When you get near the end, you will have to disassemble the tool and set it up like it was for extraction to pull the bushing in the last 1/4 of an inch.
