You will have to remove the entire door trim panel. Not that difficult:
Gently pry the mirror adjusting switch away from the door latch. Disconnect and put aside. Underneath the switch are a couple of screws. Undo these and pull the trim panel from underneath the door latch.
Next, gently pry out the round trim plugs from the top and bottom of the door grab handle itself. Under these plugs are a pair of screws. Remove them.
At the leading and trailing edges of the map pockets are two plastic caps covering screws. Snap off the caps and remove these screws.
Gently pry the bottom edges of the door trim away from the door using a spatula or the like. This should release the press studs that secure the panel to the door.
Finally, roll down the window and gently rock the entire door trim panel back and forth. You're looking to unseat the top edge of the door trim panel from the top of the door itself. It's retained by being essentially wedged down in there.
Once the door panel's off, you can remove the plastic trim piece from the mirror, unscrew it, disconnect it, and follow the reverse of these steps to reassemble.
