The shift knob has to come off to remove the boot. On earlier cars, the knob pulls right off. It's just a pressure fit onto splines on the shaft. However, starting sometime around 91, Saab, having heard that these knobs could be removed without necessitating replacement, and being concerned about slumping knob sales, and the overall health of the knob industry, started gluing the knobs onto the splines.
The official method to remove the knob is to cut it off following a specified pattern. This method is extradinarily reliable both is removing the knob and in assuring steady knob sales. The non-sanctioned method is to put the gear in reverse, kneel on the passenger seat and pull up in the direction the knob faces while in reverse. This method is less reliable both in removing the knob and in assuring that you will in line at the dealer parts counter with $35 for a shift knob. You might also consider wearing a helmet since the knob can let go quite suddenly.
