The really high NOx looks like a mixture or ignition problem to me. Not much you can do about ignition except put the extact right NGK's in, gapped to spec.
When you pull the spark plugs, see how they look. Are they white? Are there little fused bits stuck to the electrodes? Are the electrodes eaten away? How old are the plugs.
Prematurely worn, white/burnt plugs are a sign of a lean mixture and high combustion temperatures. A lean mixture can be due to a bad O2 sensor, but you've replaced that. Next would be a bad/leaky FPR, vacuum leak, or a problem in the intake air mass sensing system. This would be a bad MAP sensor or a bad Intake Air Temp sensor. The MAP and IAT work together to determine how much air is coming in. If either one is off a bit, you could be running lean. They can be off and still keep the Check Engine light from kicking on. Check Townsend for some guidance on troubleshooting.
