I am having the same problem with one of my rear parking brakes not holding. After trying to adjust that side's rear caliper via the internal 4 mm hex adjuster screw, I determined that it is stripped/broken - I could adjust it for one engage/release cycle then it wouldn't work again, and the mechanism felt stripped. Apparently, forcefully retracting the caliper for new pads without using this screw leads to broken adjusters. Car is new to me, so I have to blame some prior work. The only repair is by replacement, and rebuilt calipers are about $60.
