Hi Steve,
I have the exact same situation on my '87 9kT. I have never heard of anyone else having it, so it is very interesting to see your post. One day four years ago it just started doing it. I too set base idle, check the TPS, tried a different AMM, cleaned the AIC, triple checked for vacuum leaks, and everything else I could think of with no luck. The Townsend site has great info on setting idle and such if you haven't checked it out yet.
I couldn't bare the sound of my 9k cold-starting at 2000rpm, so I installed a switch inline with the AIC using a spare fog light switch. This way it cold-starts at base idle (with a minimal rev), I lightly press the throttle for a couple second after start up to maintain normal idle, then push the switch and go.
It a terrible solution really, but the best I could come up with. Please do let me know if you figure anything out as I would love to properly correct the situation!
