The tensioner is a spring device and it will be difficult to detect wear or fatigue just by looking at it. At a minimum, you will have to buy a new o-ring and and sealing ring if you take out the tensioner to roll in the chain. It will add $30-$40 to the job to buy a new tensioner, but you will reduce your worry by at least that much. Also, depending on the year of your car, there was a new style tensioner introduced (1988?) that worked better than the old style.
