AMM/ECU and hesitation probs?

I am dusting down my 2nd 9000, it is a UK 1990, 2.3 Turbo S, manual, TCS, LH2.4 with lambda and non adjustable AMM. The car had been parked for several years before I got it and am bringing it back to work!

I am experiencing a couple of niggling probs though and would appreciate any help.

The CEL is on; Fault codes read are:

12111 Learning fault in lambda(air fuel mixture adj) vacuum leak,oxygen sensor,air mass meter
12112 Learning fault in lambda(air fuel mixture adj) vacuum leak,oxygen sensor,air mass meter
12223 fuel air mixture too lean vacuum leak,oxygen sesor,air mass meter
12241 injector faulty

On accelarating, the car will reach around 3000rpm and seem to hesitate for a fraction of a second - like a flat spot - then pick up. It is minor, but CEL light and this sympton make me want to get it sorted.

I have done the following:
Checked for leaks - all vac lines are ok. Did discover a busted (hooter) air bypass valve though. the car drives more smoothly since replacing with the good one from my 2.0T.
Checked all wiring from AMM to ECU.
Checked AMM output - it around 2.3V to 3v+, however, I do not see any burn off! I have not burn off voltage indicated and have looked at the hot wire and cannot see it glow at all. Could this point to ECU or AMM?

AMM BOSCH part number ends in 012. I have a spare but it is for a 2.0T and ends in 017 and from reading the board, I don't think is interchangeable.

I have taken each of the injectors out and cleaned them thoroughly.

The DI cassette is only 10K miles old and the plugs are new. Tried another DI and car was the same.

I have not checked fuel pressure or replaced the fuel filter yet.

Any ideas on what to do next?
