First, the fault codes:
675BO=ETS ECU fault: Calibration not completed
E7590=Safety system activated
As for the symptoms, it's hard to say if they mean anything. So many things can throw you into limp home mode. I assume that all of your vacuum hoses were checked. The other cheap thing you could do is to clean the electrical connections. Still, it sounds like you either have a bad throttle or ECU. ECU's are pretty hardy, so I'd think bad throttle. It's tough to test a throttle when the problem is intermittent, but you might be able to see something if you can check it right when it goes into limp home.
Look on the Townsend imports website. They have a page with ETS checks, and the throttle body resistance tests could tell you something. It does seem funny that you didn't get any codes related to the throttle... Maybe I should leave this to Anders.
I had a bad throttle, and I feel like I remember incidents like you describe where even the backup cable for limp home stopped working. I'm not sure though.
