What you are supposed to do with the r134a adapters is to remove the valve cores from the r12 service ports altogether and then screw on the r134a adapters. There have been cases where the r12 service port valve cores were not removed and the refrigerant escaped around the coupler and never made it into the system.
It sounds like you might have a problem with the relay to me. However, I once accidentally swapped the electrical connector for the pressure switch with the electrical connector for the cabin fan speed resistor on the front of the evaporator housing (They look the same) and I had no ac and no cabin fan. If you removed the evaporator, you might want to check that also.
The anti-frost switch is on top of the evaporator box under a small plastic cover.
