I haven't used Catco, but one of my clients is a company that makes cats for the OEMs and I can hopefully help. Trouble with your cats premature failure can occasionally be due to mfg defect, but twice seems odd. Usually cats fail because there is some other problem with the car running too rich, burning stuff it shouldn't burn, or something of the like. Could be faulty O2 sensor, Mass airflow sensor, temperature sensors, or anything else to do with fuel air induction, or incomplete burning of the fuel.

Most common cause of failure of the cat is contamination from too much unburned fuel or even raw fuel. WHen it then gets hot, it basically burns out the inside of the cat. THis burning basically renders the catalyst inside your cat useless so it cannot perform its duty of changing the chemical makeup of your exhaust into non-polluting stuff. Sometimes the material inside the cat will break up and blow out the rear. Of course this messes up your O2 readings if your O2 sensor is aft your cat (don't know what model you have) and give you fault codes and often a CEL.

I would try to get your cat replaced under warranty, but look upstream for problems before you install a new one or you will be doing it over again.
