If the upper mount is shot, then the lower mounts probably are, too. there are three lower mounts- one under the tranny, and two under the engine. I personally haven't had any tranny mounts, go, but I've definately had the lower engine mounts fail.
Expect to pay $350-$500 to have the two lower mounts replaced. The lower price would be a good independent, the higher would be a dealer. You can do lowers yourself for about $150 in parts and about 2-3 hours of time. There have been some very good writeups on this.
The upper mount requires a new bushing be pressed in. There are two bushings - one mounted on the 'dogbone', and one mounted on the engine. They need to be pressed out, and a new one pressed in. The dogbone is easily pulled, and the pressing can be done by any decent shop with a press. The mount on the engine requires some tools. A dealer can do it, or you can make up tools yourself from a handy hardware store - large washers, and some threaded rod.
I recommend using the OEM hydraulic mounts for the lowers, and OEM rubber mounts for the upper. Some folks use solid rubber (cheaper) or poly (stiffer, not as cheap) instead of stock. Those might improve handling and shifting, IF you've made other suspension mods. However, there is an increased chance of vibration, sometimes severe.
