We will need the model year and engine type to start, e.g., 1990 2.0 liter turbo 5 speed.
But, to answer your question, the symptoms would be the car running either very rich (black smoke out of the exhaust is one) or very lean (car runs hot and pings/low boost). You would test the regulator by tapping into the fuel rail and measuring the fuel pressure.
The most common failure of the FPR is a leak in the diaphragm, which causes fuel to enter the vacuum line directly into the intake manifold. This causes a too rich condition. The test for this condition is to pull off the vacuum line and see if fuel is leaking through the regulator.
FPR failures are a relatively rare occurrence. Start with the ignition system, then check the other parts of the fuel injection system next. Depending on the year of the car, and air leak or a bad AMM can cause problems like you describe.
