12 volts on which wire? On a '94 it should be a 3 wire sensor. The 2 white wires are for heating the sensor and would measure 12V but have no influence in fuel management once the engine warms up. The single black wire carries the signal to the ECU. You can check the signal with an analog voltmeter (one with a moving needle - with a digital meter the numbers change too rapidly to read).
You connect the positive lead of the meter to the black wire that comes out of the sensor, and the negative lead of the meter to a good ground. With the car at idle, the voltage is cycling from just over 0V to a little under 1V every few seconds. If the needle doesn't move or the voltage range is less than about 0.6V, the sensor needs replacement. (You need to use the most sensitive or low range in the meter ideally 1 volt full scale.

BTW - I've seen in automotive stores some fuel additive you pour in the gas tank before a test that claims to clean the emissions and pass the test or your money back. Haven't tried it but worth a try.
