My understanding is that the Engine Control Unit (ECU) disregards the oxygen sensor for the first few minutes of operation, as the reading is guaranteed incorrect until the sensor is heated up.
How about a vacuum leak caused by a disconnected or cracked 12-year-old rubber vacuum hose? I have replaced cracked hoses on several cars younger than that, including my 1994 9000. How about the large rubber grommet where the PCV line plugs into the valve cover - this gets mushy and the fitting could fall out? A cold engine will receive extra fuel from the ECU on initial startup, then run too lean after it gets the normal fuel volume coupled with a vacuum leak. These should be easy enough to check if he's at all mechanically inclined.
