If you have alloy wheels that have open spaces so you can see the calipers, look in with a flashlight. Look for the brake pad - there is a slightly wider metal piece, and the pad rests right against the rotor. If the pad is less than 1/4 inch thick (about 5-6mm), the pads need replacement.
If you can't see through the open parts of the wheel, then follow Yaofeng's suggestion - jack up and remove the wheel, and inspect the brake pad.
How much to replace? If the rotor is in decent shape, about $60-$80 to replace the pads. I'm not really sure - I do my own brakes. Brakes are very important, and they are one of the easiest things to do on a car. You can get a set of pads for about $35 - that does both sides (always do both sides at the same time). If the rotors are shot, then they cost about $40 each to buy.
