Like many main pulleys these days, the pulley is actually a sandwich. There is the center part, which bolts to the crankshaft. The outer portion of the pulley, the portion that carries the belt, is connected to the inner portion by a thin rubber section. Over time and abuse (like oil exposure, heat, etc.), the bond between the rubber and metal can fail, or the rubber itself fails. Now, the outer portion of the pulley can turn independent of the inner (drive) portion. Usually there will be just a little relative motion on start-up, and like a clutch, once things get going, the parts stay together. But there is some slippage. If the outer portion of the pulley starts to drift backwards, it can start grinding into the oil pump housing. If it drifts outwards, it can come off. It also mis-aligns the serpentine belt.
It's called a Harmonic balancer because the rubber piece helps isolate vibration, and contributes to quieter, smoother operation. Most cars have this setup.
